Having the right information with you can help your child’s physical examination go smoothly and make it easier for the medical staff to complete any required paperwork.
School or sports physical forms: If your child’s school or sports program provided a specific form to be filled out, bring it with you. Our team can complete the required paperwork during the visit.
Immunization records: Though helpful we do not require them. Bring your child’s immunization records if you have them available.
Medication information: Make a list of any prescription medications, over-the-counter medications, or supplements your child takes regularly.
Relevant medical records: If your child has medication or significant food/environmental allergies, an ongoing health condition, a history of surgery, or other significant medical history, bring any records or information that may be helpful.

What If I Don’t Have All of My Child’s Medical Records?
You should still bring your child in for their physical even if you do not have medical records available. Bringing whatever relevant recent or prior health information you do have is helpful but not required.
